Methods and Standards for Establishing Payment Rate; Other Types of Care (12VAC30-80)
In accordance with Item 291 JJJJJ 4 of Chapter 1 of the 2026 Acts of Assembly, Special Session I, the state plan is being revised to increase the rates for private duty nursing services under the early and periodic screening, diagnostic, and treatment (EPSDT) benefit by 5.0%. A corresponding rate increase will be provided for these services under home and community- based waivers. These increases are not included in the state plan amendment but via waiver documentation.
The expected increase in annual fee-for-service aggregate expenditures is $27,289 in state general funds and $27,718 in federal funds in federal fiscal year (FFY) 2026 and $165,778 in state general funds and $165,911 in federal funds in FFY 2027.

Methods & Standards for Establishing Payment Rates; In-Patient Hospital Care (12VAC30-70)
In accordance with Items 291 BB 6 and BB 7 of Chapter 1 of the 2026 Acts of Assembly, Special Session I, the state plan is being revised to maintain the rate ceilings that were in effect as of June 30, 2026, for psychiatric residential treatment facilities (PRFTs) and participating addiction and recovery treatment services (ARTS) residential services. Effective July 1, 2026, DMAS shall eliminate rebasing and inflation adjustments for PRTFs and qualifying ARTS residential services providers.
The expected decrease in annual fee-for-service aggregate expenditures is $428,179 in state general funds and $486,363 in federal funds in federal fiscal year (FFY) 2026 and $3,044,313 in state general funds and $3,352,203 in federal funds in FFY 2027.

In accordance with Item 291 PPPPP of Chapter 1 of the 2026 Acts of Assembly, Special Session I, the state plan is being revised to eliminate inflation adjustments for hospital rates, freestanding psychiatric facilities, disproportionate share hospitals (DSH) payments, graduate medical education payments, nursing facilities, and any other provider rates for fiscal year (FY) 2027 and FY 2028. DMAS shall exclude the value of withheld expected FY 2027 and FY 2028 inflation adjustments from future inflation adjustments and hospital and nursing facility rebasings.
The expected decrease in annual fee-for-service aggregate expenditures is $1,697,130 in state general funds and $3,011,087 in federal funds in federal fiscal year (FFY) 2026 and $12,600,620 in state general funds and $22,386,610 in federal funds in FFY 2027.

Methods and Standards for Establishing Payment Rates; Other Types of Care (12VAC30-80)
In accordance with the Virginia Medicaid State Plan (Supplement 6 to Attachment 4.19-B) and 12VAC30-80-300, the state plan is being amended to update the average commercial rate (ACR) calculation for supplemental payments for services provided by physicians at Virginia's freestanding children's hospitals effective July 1, 2026. The updated ACR percentage of Medicare will be 218%.
The anticipated fee-for-service expenditures are $357,639 in state general funds and $363,262 in federal funds in federal fiscal year (FFY) 2026, and $1,441,225 in state general funds and $1,442,378 in federal funds in FFY 2027.
